    New Imaging Platform Boosts Accuracy During Complex Spine Surgery Dr. Gururaj Sangondimath

    Kamlakar, a professional flautist, was on the verge of bidding his career goodbye. The 55-year-old had started to experience a progressive weakness and difficulty while moving his fingers. The problem had become so bad, he had to stop playing the flute. He was later diagnosed with cervical myelopathy, resulting from compression of the spinal cord in the neck and advised surgery by specialists.

    Surgical intervention for Kamlakar required cervical pedicle screw fixation, a procedure fraught with risk of instrument malpositioning. One wrong move could potentially disrupt the entire neuro-vascular system. The spine surgeons, therefore, decided to use an advanced surgical imaging platform - an O-arm navigation system - to perform the surgery. Kamlakar has now returned to playing the flute.

    We used O-arm navigated spine-fixing surgery to treat him. Our study has shown the imaging platform has 99.5% accuracy while performing complex surgeries. This has significantly reduced risk of neurovascular injuries," said Dr Shailesh Hadgaonkar, the chief spine surgeon at Sancheti Hospital - one of the dedicated centres for orthopaedic surgeries in India.

    The study was recently published in the international journal Interdisciplinary Neurosurgery. The spine team at Sancheti hospital, headed by Dr Hadgaonkar, has performed a number of surgeries using the novel technology.

    "The O-arm surgical imaging system is a platform designed for use in spine surgeries. It provides real-time imaging of a patient's anatomy with high-quality images and a large field-of-view in both 2-D and 3-D," Dr Hadgaonkar said.
    O-arm navigation technology essentially functions like a GPS system that enables the surgeon to place the instrumentation at the accurate position in the small mass of the pedicle bone. It increases precision as compared to 2D C-arm imaging and helps prevent any complications of neurological damage or malposition of the screws.
     
    Pedicle screw fixation in the osteoporotic spine is a challenge if bone quality is poor, leading to various complications such as screw backout, loosening and pedicle breach.
     
    But with the advent of the O-arm navigation, it is now possible to place pedicle screws with very high accuracy. Besides, patients needing spinal surgery for spine deformities including scoliosis, kyphosis, spine tumor, spine fractures, spinal stenosis, spinal instability, and various other spinal conditions can also greatly benefit from the robotic technology, experts said.

    Delhi-based Dr. Gururaj Sangondimath - Senior Consultant Spine Surgeon in Delhi, with the Indian Spinal Injury Centre, said, "The O-arm technology is the most advanced one in spine surgery and allows for the most accurate placement of spine fixations. It has made spine surgery far safer."
     
    Dr Nirali Mehta, an associate surgeon, said, "Earlier, we would do a procedure called lateral mass screw fixation. But cervical pedicle screws offer superior fixation and with O-arm navigation, the chance of instrument malpositioning is next to nil."
     
    Over the past decade, pedicle screw fixation of the spine has gained greater acceptance as a result of improved devices and technology. Nevertheless, accuracy of screw placement remains a concern, especially in training institutes where expertise in screw placement may be limited or not available to all surgeons.
     
    "C-arm fluoroscopy is commonly employed for intraoperative anatomical orientation of screw placement. Intraoperative fluoroscopy and serial radiography only demonstrate the depth of screw penetration, but cannot be used to recognise screw malpositioning. The O-arm imaging system has addressed these gaps," Dr Hadgaonkar said.
     
    The system is not without some limitations. Cost of this emerging technology remains a deterrant. Another key limitation is surgeons will need considerable training, experts said.

    We would like to share a case from Delhi. A girl aged 26 went swimming and as she dived in, she had an injury to her neck. Because of this injury, she had paralysis in the body below the neck. Now, she is undergoing the treatment at Spinal Injuries Center in Delhi by Dr. Gururaj.

    • While diving, please check the depth of water to avoid injury by colliding with the bottom of the swimming pool. 
    • Follow Foot first dive and avoid head first dive.
    • Such incidents happen in people of age 15 - 45 as people try to do adventures at this age.
    • Even if such incident takes place, it's necessary to lift entire spine of patient as a single unit.
